About the Project
Malvern Drive Supported Living Service project has twelve self-contained flats all within the same building, eleven are for individuals who experience serious mental illnesses, and the twelfth is the staff office, communal space and accommodation for sleep-in staff.
The Service provides each person with an individual care and support package. The support to individuals is to enable each person to be as independent, active and as safe as possible, whilst promoting and protecting their human rights and affording them the very best quality of life.
Malvern Drive Supported Living Service staff will work alongside other professionals, Mental Health services, families, carers and other organisations to provide an outcome-based service to promote well-being and recovery, which focuses on the needs, aspirations and priorities of each occupant.
The Service will give the individuals opportunities to develop their independent living skills, to maintain their tenancies.
The project is staffed 24/7, including a sleep-in staff member.
Eligibility / Referral Process
Adults experiencing mental ill health who, following a wellbeing assessment, have been identified as requiring care and support.
Referrals are identified by the Cardiff Social Care Team and Community Mental Health Team. They will provide Adferiad Recovery with the individual’s details, up-to-date Care and Treatment Plan, Risk Assessment and Well-Being Assessment.
During the assessment for move-in process the Adferiad Recovery service staff will work with the identified individual, the Complex Placement Team and Community Mental Health Team to ensure that needs are identified and the transition into the supported accommodation can be managed in a way that best meets those needs.
