About the project

The service delivers activities which are designed to support the improvement between North Ceredigion CMHT and the local community.
We work in partnership with health board colleagues and other agencies to improve the practical, emotional and social outcomes of the individual.
We aim to reduce the experience of social isolation, improve stability and support improvements in physical health. The service works to empower individuals to better manage mental health conditions through education, information and support.
The service provides support in varying ways through:

  • Unplanned, walk in support
  • Delivery of workshops
  • Advice and guidance
  • Recovery Plan sessions
  • Skills development such as self-management, coping strategies, confidence building
  • Carer and Family Support.

We provide both 1:1 support; workshop and group activities which strengthen skills, and which can be used to promote social inclusion; provide individuals with opportunities to build relationships, develop communication and social skills and build confidence.
These workshops include:

  • Walking Groups which aim to support the improvement of physical health whilst also providing an opportunity for meaningful social interaction
  • Healthy Living Groups which provide support to plan and cook healthy, well-balanced meals.
  • IT sessions which provide support to develop skills and confidence in using equipment safely.
  • Information, advice and signposting sessions which provide individuals with effective connection to community services.

The service is funded by Hywel Dda Health Board.

Eligibility / Referral Process

Mental health.
All referral routes are considered and include CHMT; GP surgeries; voluntary sector; the police; local university; via carers, and, also through self-referrals.
