About the Project
The Gorwelion Link Worker role is a service based in Aberystwyth that operates from Gorwelion Day Hospital and Portland Road between 12 noon and 4pm on Fridays. Our service empowers individuals to better manage mental health conditions by providing education, information, and support. We aim to reduce the experience of social isolation, improve stability, and support improvements in physical health for each individual. We work in partnership with health board colleagues and other agencies to improve the practical, emotional and social outcomes of every individual that we support.
Who do we support?
We provide support to individuals aged 18 and over who are residents in the Ceredigion area and are struggling to manage their mental health. They may be experiencing psychotic disorder, low mood, or problems with anxiety and stress. We offer unplanned, walk-in support, as well as the delivery of workshops and skills development. We also provide support to unpaid Carers, offering advice, information, and signposting.
How do we do it?
We provide one-to-one support as well as workshops and group activities which strengthen skills, promote social inclusion, provide opportunities to build relationships, develop communication and social skills, and build confidence. These include walking and healthy eating groups, IT workshops, as well as advice, information, and signposting. Altogether, the support we provide aims to improve physical health whilst also providing an opportunity for meaningful social interaction.
Referral
We accept referrals from GPs and the secondary sector, as well as from third-sector agencies and carers. Self-referrals from individuals are also accepted.
If you would like to discuss the referral process with us or request more information, then please get in contact using the phone number or email address above.
