About the Project
The service operates under the Supported Living Service Framework; Ceredigion Council send out EOI (Expressions of Interest) and providers review if they can meet the needs of the client and have the staff capacity to support them. A Support Plan is then submitted as an Expression of Interest / Bid and the application is assessed by the Panel / Care Coordinator and a provider is chosen for the Individual Package. Individuals who are placed on this Framework require various types of support to enable them to remain living independently in the community.
The service supports individuals to:
- Attend appointments
- Pay bills / Deal with other financial matters
- Interact with the community by encouraging & enabling them to feel part of their community including reducing isolation and building resilience
- Build up friendships with their peers
- Attend groups like art and music and explore new avenues of opportunities
- Signpost to other services when needed
- Work on Independent Daily Living Skills
- Create meaningful occupation and community involvement via volunteering

Individuals with mental health requirements
Referral routes include CHMT/Health Board/HSG/ EOI’s (Expressions Of Interest) Local Authority.
