About the Project
Support may include:
- Assessment review
- Risk assessment and management including Safeguarding
- Care planning including monitoring and review
- Liaison work e.g. primary care, Social Services and Mental Health service providers
- Discharge planning and management.
- Pre and post detoxification/residential rehabilitation support
- Relapse prevention
- Referral to self-help groups
- Advice and information
- Regular support as agreed in care plans
- Structured counselling sessions provided by qualified counsellors

Any person aged 18 and over who requires support with Drugs and/or Alcohol
