About the project
Support may include:
- Assessment review.
- Risk assessment and management including Safeguarding.
- Care planning including monitoring and review.
- Liaison work e.g. primary care, Social Services and Mental Health service providers.
- Discharge planning and management.
- Pre and post detoxification/residential rehabilitation support.
- Relapse prevention.
- Referral to self-help groups.
- Advice and information.
- Regular support as agreed in care plans
- Structured counselling sessions provided by qualified counsellors
