Llys Glan yr Afon, Newtown – Extra Care Scheme
Adferiad works in partnership with Powys County Council (commissioning body), and the Housing Management agency West Wales Housing Association (WWHA) in the delivery of on-site 24-hour care and support of individuals living within the purpose built 48-apartment development, Llys Glan yr Afon.
Extra Care schemes, such Llys Glan yr Afon, provide individuals with an alternative to residential care, nursing care or sheltered housing, enabling individuals to live in their own home within the site, with their own front door. It aims to provide ‘a home for life’ even if an individual’s needs change over time.
Adferiad is responsible for providing the support and care element of services provided within Llys Glan yr Afon. This is provided in individuals’ own homes to support their wellbeing and independence, and meet the needs and changing expectations of individuals, allowing them to live as independently as possible in their own homes.
Support is provided 24/7 on both an ADHOC basis (via call bell system accessible in everyone’s home) and on a regular basis (commissioned visits), such as twice daily or more frequently if required and are based on an individual’s assessed needs, desires, and personal preferences to promote wellbeing and achieve their identified outcomes. This includes:
- Support with Personal Care.
- Support with Nutrition and Hydration.
- Support with Prescribed Medication.
- Support with Mobility.
- Promoting opportunities for social inclusion.
This service includes working in partnership with local community nursing teams, other health provisions and other providers of support.
Eligibility / Referral Process
To be considered for an apartment at Llys Glan-yr-Afon you must be:
- Aged 18+
- Have a care and support need
Priority is given to people already living in Powys.
