About the Project

Adferiad works in partnership with Powys County Council (commissioning body), and the Housing Management agency West Wales Housing Association (WWHA) in the delivery of on-site 24-hour care and support of individuals living within the purpose-built 48-apartment development, Llys Glan yr Afon. Extra care schemes, such Llys Glan yr Afon, provide individuals with an alternative to residential care, nursing care, or sheltered housing, and enables individuals to live in their own home within the site. It aims to provide ‘a home for life’ even if an individual’s needs change over time.

Who do we support?

To be considered for an apartment at Llys Glan-yr-Afon individuals must be aged over 18 and have a care and support need. Priority is given to people already living in Powys.

How do we do it?

Support is provided 24/7 on both an Ad-hoc basis (via the call bell system accessible in everyone’s home), and on a regular basis (through commissioned visits), such as twice daily or more frequently if required. The service also works in partnership with local community nursing teams, other health provisions, and other providers of support. Support is based on an individual’s assessed needs, desires, and personal preferences in the aim of promoting wellbeing and achieving their identified outcomes.

This includes:

  • Support with personal care
  • Support with nutrition and hydration
  • Support with prescribed medication
  • Support with mobility
  • Promoting opportunities for social inclusion

Referral

To be referred into the service you must be aged 18 or over and have a care and support need. Priority is given to those who live in Powys.
