About the Project

Llanelli Housing is a 24-hour Domiciliary Care, Temporary Supported Housing service which is designed to meet the needs of those who experience serious mental illness (SMI), have complex needs and require medium levels of support. Occupants are supported to learn the skills to enable them to move on confidently to independent living. These skills include maintaining a tenancy, learning their relapse signs, gaining the confidence to ask for help and finding meaningful activities. This service is situated in Dafen, Llanelli, a short bus ride away from town.

Who do we support?

We support individuals who experience serious mental illness; the lead need of referrals must be serious mental health, although very often referrals have co-occurring needs including drug or alcohol addictions, ASD or a learning disability. Depending on needs, each occupant with have a comprehensive support plan in place which is reviewed a minimum of every three months.

How do we do it?

The recovery process is managed using Adferiad’s Support Plans, Wellbeing Wheel and Housing Support Grant (HSG) Outcomes. These are used to promote empowerment and self-management, a commitment to progress and apply a whole-person approach. The scheme supports occupants to establish a stable, safe home, and develop life skills in the community to maximise their opportunity for independent living. It is anticipated that people will move through the Project as part of their journey towards independence into their own home.

Referral

To be eligible to live in Llanelli Housing you must be under the Community Mental Health Team (CMHT) with a lead need of serious mental illness.

The referral process is as follows:

The Care Coordinator (Community Psychiatric Nurse (CPN), Social Worker), must complete a Housing Support Grant Assessment (to identify the person’s support needs) and a completed referral form, all to be submitted to Carmarthenshire Housing Support Grant Team. The HSG Team will assess and decide whether the individual referred is to be signposted to Llanelli Housing.

The Care Coordinator can contact Llanelli Housing Service Manager to discuss the proposed referral. Relevant paperwork will be required as part of the referral process.

Carmarthenshire County Council Panel and Adferiad Recovery Housing panel will assess the individual’s support needs (including meeting and assessing the individual referred). Adferiad Recovery’s Llanelli Team will arrange for the referred person to visit the supported accommodation to see the project.

If and when the decision has been made for placement, The Adferiad Recovery Service Manager / Registered Manager will contact you and your Care Coordinator to meet and organise a move-in date.
