About the Project
The Gwent Carers Information and Advice service is based on the adult and older adult wards across Gwent. The long-term outcome that the project aims to achieve is to improve the wellbeing of carers and other family members, and ultimately impact positively on the health of the service user. In addition, we want unpaid carers to feel equipped with the necessary skills to support their caring role and to be able to consider their own needs.
Who do we support?
The service supports carers and/or families of individuals with mental health issues, or dementia.
How do we do it?
Staff facilitating the cafes provide a range of information and advice relevant to carers. This includes information on mental health conditions, mental health legislation, training opportunities, Carers Assessments, and benefit entitlements.
The outcomes we aim to achieve include:
• Carers to feel informed and skilled
• Carers are involved in the planning and delivery of care
• Carers are aware of their rights and the support available to them
• There is a reduction in social isolation and access to peer support networks
• Carers feel empowered and engaged through having a voice in the planning of services and care
• Carers feel valued for the role they play and have a sense of purpose
• Carers are able to consider their own needs
Referral
We cover all five counties across Gwent, and the carer needs to be aged 18 or over. Referrals are accepted from the carer and ward staff as well as mental health teams in the community.
