About the Project
The Carers Discharge Support Service and the Community Outreach Support Service offer support to carers of inpatients in Withybush General Hospital and the South Pembrokeshire Community Hospitals. Our service ensures that the carers voice is heard through the discharge planning process and after patient discharge by providing information, advice, signposting, and referrals. The Community Outreach benefits carers by helping them access support within the community to help maintain their own health and wellbeing as a carer.
Who do we support?
We support adult carers (18+) who are caring for an inpatient of Withybush General Hospital or South Pembrokeshire Community Hospitals. The Carers Hospital Discharge Support Service can support with conversations, input to patients care, and discharge planning by supporting the carer in planning meetings. The Community Outreach Discharge Support service is available after patient discharge; it provides a wide range of community-based information, advice, signposting and carer assessments.
How do we do it?
The support we provide is bespoke and person-centred around the carer and the carer’s needs. We offer information, help and advice to carers throughout the hospital discharge process. The Carers Discharge Officer is also able to attend discharge meetings with the carer, and the Community Outreach Discharge Support Staff is able to visit the carer at home after hospital discharge to ensure support is in place and provide carers with further support.
