About the Project
Pembrokeshire Supported Living Service is a Registered Domiciliary Care Service providing support across Pembrokeshire to assist people with disabilities or dependencies to live in their own homes. We focus on enabling service users to maximise their independence by providing support in areas such as personal care, daily living activities, and other practical tasks. Service users will be encouraged to develop the confidence and skills to carry out these activities and tasks for themselves.
Who do we support?
We provide community-based, person-centred supported living services to users who have a learning disability or a physical sensory disability, or who experience mental health illnesses and may have additional disabilities and physical conditions as a result.
How do we do it?
As a Registered Domiciliary Care Service, we deliver support to enable people with disabilities or dependencies to be able to live in their own homes, achieve their own individual aspirations, and work towards achieving their goals and independence. The support we offer includes the maintenance and improvement of users’ physical, mental health and emotional wellbeing; access to sport and leisure, arts and culture; learning skills to prepare for volunteering or employment opportunities; and increased social opportunities and maintenance of contact with people who are important to them.
Referral
We offer support to adults 18 years of age and above across the county of Pembrokeshire. Referrals or enquiries are to be made to Pembrokeshire County Council SLS Team. Enquiries are to be made to Pembrokeshire County.
