About the Project

The Community Connectors are a team of dedicated practitioners, based within the NHS GP cluster areas of Cardiff City and South, Cardiff South East and Cardiff East, with an office based on Neville Street in Riverside, Cardiff. Our team is available for any patients from within these areas who would benefit from social and wellbeing support. Our staff are out and about, visiting each GP practice in these areas and supporting people who may otherwise find it challenging to access local support services.

Who do we support?

Community Connectors support clients who may need emotional, social, or mental health and wellbeing support. The team will be available to work with patients to identify areas where they can make improvements to their lives, and offer support by directing them to appropriate and relevant local community groups, support networks and other services that will benefit people’s social needs and their wellbeing.

How do we do it?

The support provided by the Community Connectors team can work instead of, or alongside health provision. We understand that people may have cultural and other needs, and our service is there to understand the individual to find out what their support requirements are. Acting as a bridge for people to access community and peer support services, resources and opportunities, the project can support people to make better personal connections to help with combating loneliness. Our community connectors can support people to discover local services and can empower them to make informed decisions about their needs, building on the skills that they already have. By applying a strengths-based service approach, our service supports a more resilient community.

Referral

To access one of our community connectors, people need to be registered with one of the 18 GP practices within the Cardiff City and South, Cardiff South East and Cardiff East  cluster areas.

You can then either ask your GP surgery to refer you or contact us using the contact details above.
