About the Project
The Denbighshire Collaborative is a trusted and experienced partnership jointly delivered by Adferiad and Stori who have over 20-years of experience working in Denbighshire and over 40 years across Wales. The service is funded by Denbighshire Housing Support Grant. Denbighshire Collaborative Service works in Partnership with Stori with Adferiad being the lead organisation.
The Denbighshire Collaborative delivers a new, holistic, housing-related support project designed to support people living in Denbighshire to sustain their accommodation and prevent homelessness. The support provided will be flexible and person-centred, supporting people to maintain sustainable housing by addressing any mental health, substance use or any other problems they may face, helping to improve their health and wellbeing.
Who do we support?
Individuals supported will be any gender; aged 16 and above; single or part of a couple and may or may not have dependent children. Service users may be residing in a property that is privately rented, social housing or owner-occupied. The primary support provided will be housing-related support. However, this will also include any other support needs that are identified, including, but not limited to, mental and physical health and addiction needs.
How do we do it?
Support is delivered between the hours of 9am-8pm Monday – Friday and 9am-5pm on a Saturday. There is also a staff member on call in the event of an emergency outside of these working hours. The support provided will be flexible and person-centred, supporting service users to maintain sustainable housing by addressing any problems they may face, helping to improve their health and wellbeing and/or helping them progress into, or nearer to, a job or training opportunity based on their specific circumstances. The support provided in this project may also include budgeting, including accessing money advice; developing life skills; achieving safety and security, and accessing other services and opportunities with the aim of making homelessness non-recurring.
Referral
To access the Denbighshire Collaborative Service, all individuals must refer themselves or be referred to the Denbighshire County Council Pathway. All referrals need to be living in the County of Denbighshire and need to be aged 16 and over.
