About the project
Monmouthshire Carers Service provides a flexible outreach service across Monmouthshire for adult carers of adults with a mental health illness; enabling carers to develop the skills and confidence to care and to help them have a life of their own.
The service offers support with:
• Information and Advice
• Access a Carers Assessment
• Signposting to other sources of support
• Respite opportunities
• Advocacy support
• Emotional support
• Carers Group
• Access to training opportunities
Eligibility / Referral Process
All carers of individuals 16+ with a mental health diagnosis.
Open referral process. Referrals accepted from Primary and Secondary Care, self referrals, family, social services
