About the Project
Adferiad’s Monmouthshire Carers Support is funded by Monmouthshire County Council and provides a flexible outreach service to carers across Monmouthshire. The service offers support, advice and wellbeing opportunities for carers who support someone with a functional mental health need. We aim to help promote carer resilience and provide respite opportunities from their caring role. Support can be offered on a one-to-one basis, or in groups. Staff can also meet with the carer in the community, local hubs, or the carer’s home.
Who do we support?
We offer support to all carers of individuals aged 16 and over with a functional mental health diagnosis, and where the cared for lives in Monmouthshire.
How do we do it?
Our service offers support for carers in their caring role to reduce carer stress, help promote carer resilience, and offer respite opportunities from their caring role. This allows them to continue to support the cared for.
Support offered is based on the needs identified by the carer and includes:

▪ Information and advice
▪ Help to access a carers assessment
▪ Signposting to other sources of support
▪ Respite opportunities
▪ Advocacy support
▪ Emotional support
▪ Carers Group
▪ Access to training opportunities
Referral
We have an open referral process. Referrals are accepted from primary and secondary care, self referrals, family, and social services.
