About the Project
The service is a Domiciliary Service (Previously Queen Street), providing support to the client group to live independently in the community.
The service provides support with:
- Attending appointments
- Volunteering
- Paying bills and other financial matters
- Interacting with the community, encouraging & enabling clients to feel part of the community by reducing isolation and building resilience
- Building up friendships with peers
- Attending groups like art, music and exploring new avenues of opportunities
- Signposting to other services when needed
- Working on Independent Daily Living Skills
- Creating meaningful occupation and community involvement via volunteering
The Service is funded by Hywel Dda Local Health Board, HSG Local Authority

Individuals with mental health requirements
All referral routes include CHMT/Health Board/HSG
