About the Project
Adferiad Domiciliary Specialist Mental Health Project staff team are based in Portland Road, Aberystwyth. The project’s support is designed to meet the needs of those who experience serious mental illness (SMI), have complex needs, and require high levels of intensive support. We offer support that enables each person to avoid hospitalization, live in their community, and at home independently. A big part of the Adferiad support role is helping the individuals accept their diagnosis and help to identify relapse indicators. We also provide support to find meaningful activities which can range from volunteer work, training, education and social groups.
Who do we support?
Ceredigion Specialist Mental Health Domiciliary Support Service is provided to individuals aged 18 years and above who have been medically diagnosed as suffering from a mental illness, typically schizophrenia or serious affective disorder.
How do we do it?
The overarching aim of the service is to enable individuals to live independently. As part of the individual’s support plan, Adferiad support workers will provide advice on managing personal budgets, maintaining support networks, and managing relationships with neighbours and families. Staff will also support individual occupants to access other services such as specialist housing and financial advice, care or treatment services. Additionally, staff will support each occupant to access, maintain and manage accommodation by assisting individuals to develop or maintain the necessary skills and confidence to live as independently as possible.
Referral
Individuals can be referred to the Brokerage Panel / Commissioners by their CMHT Care Coordinator or Social Worker, as well as by Ceredigion County Council or Hywel Dda.
