About the Service

The aim of Carmarthenshire Preventative Mental Health Services is to better support individuals with their mental health by offering tailored support that engages with users as individuals. Our trained staff go through a ‘wellbeing conversation’, focusing on what matters to users such as their individual needs, goals and aspirations. This feeds into a unique wellbeing plan that looks at all areas of individuals’ life, allowing us to provide support that’s specifically tailored to users.

Who do we support?

We support anyone with a mental health problem living in the Llanelli, Gwendraeth Valley and Amman areas.

How do we do it?

Every person we support is offered up to six support sessions which allows our staff to make sure our plan works for each individual, no matter their situation or experience. Our service offers support with: wellbeing, independence, resilience, and quality of life. We also offer a central referral route to other support via the hubs provided by the other Preventative Services-approved providers.

Referral

Anyone over the age of eighteen can be referred into the service. Referrals are accepted via self-referrals from individuals, professional referrals from GP’s and CMHT, as well as other Third Sector services via the Preventative Services Hubs.

If you would like to discuss the referral process with us or request more information, then please get in contact using the phone or email address above, or alternatively, contact charlotte.smith@adferiad.org.
