About the Project
Adferiad Cardiff Supported Living Service is a Core and Cluster model, where individuals hold their own tenancies within blocks or clusters of flats in Cardiff. Adferiad provides each person with a targeted domiciliary care and support package, to enable individuals to be as independent, active, and safe as possible and maintain their tenancy.
Who do we support?
We support adults aged 18 years and above with a primary need of mental health, who may also face social, physical, or learning difficulties, as well as challenges related to substance use, who have been identified as needing support in a number of areas to enable them to independently manage their housing tenure.
How do we do it?
We aim to provide a safe and supportive environment for individuals experiencing serious mental health challenges, learning disabilities, physical health issues and or substance use. Our service is delivered in a way to promote choice, self-determination, independence, and recovery. Support is based on an individual’s assessed needs, desires, and personal preferences with the aim of promoting wellbeing and achieving their identified outcomes.
This includes:
- Support to manage accommodation and feel safe
- Help to make choices and take positive risks
- Support to go to social or leisure activities to feel part of the local community
- Financial support and signposting
- Support for a healthy lifestyle, including food, shopping, and exercise
- Support with personal care
Referral
Our service applies to adults experiencing mental ill-health, who may also face social, physical, or learning difficulties, as well as challenges related to substance use, who, following a wellbeing assessment, have been identified as requiring care and support.
