About the Project
The service is located in Treorchy, Rhondda Cynon Taf. It is a new-build service that provides 5 self-contained flats together with a staff office and communal lounge. The service is staffed 24 hours a day and individuals have their own tenancies (Supported Housing Contracts). Our service provides a safe environment for individuals to manage their mental health and substance use. Individuals are also supported to manage their own tenancies, or to attend appointments etc. Our overall aim is to provide a safe, welcoming environment where service users can develop their independent living skills.
Who do we support?
We support individuals who have experienced mental health issues or have co-occurring mental health and substance use needs.
How do we do it?
We support people towards recovery, working with the individual and their mental health professionals to provide a service which is relevant to their needs. We support our service users to be as independent, active and as safe as possible. We do this by encouraging them to be involved in their communities, taking part in groups and activities in the local area. We also encourage people to build relationships within the community, in preparation for independent living, and we work together with individuals to manage their physical and mental health needs. We also encourage the people we support to continue to have relationships with their friends and families. When people move in, they are supported to purchase what they need for their flats, such as white goods, furniture, household goods etc.
Referral
All referrals to the service will come through the Care Coordinator or Care Manager for each individual. The service is intended for Residents of (or former residents of) Rhondda Cynon Taf.
