About the Project

The Supporting People Gwynedd project provides floating support to individuals in the county who are homeless, at risk of homelessness, or need support to maintain their existing tenancy. Staff are available 9am – 5pm Monday to Friday (with some flexibility depending on need) to provide support in clients’ homes or in the community.

The service also provides four self-contained, one-bedroom properties for individuals in housing need. These properties act as a stepping stone towards permanent accommodation.

Who do we support?

We support individuals aged 18 or over living in Gwynedd with housing-related needs. Staff work closely with clients to address any barriers to accessing housing or issues which impact upon existing accommodation, thereby reducing the risk of homelessness. Support is delivered within individual homes or at convenient locations in the community.

How do we do it?

Our support is person-centred, trauma-informed, and focuses on enabling the individual to take an active role in their journey. We work with individuals to identify the barriers and issues they are facing in terms of housing, creating a support plan to identify goals and measure progress. Support is initially intensive with visits and support sessions being delivered at least once a week and, if appropriate, will include linking in with other agencies. By supporting individuals into their own homes, or to manage and retain their existing accommodation, we minimise the impact of homelessness, both on the individual and the wider community.

Referral

Referrals are received via the Gwynedd Single Point of Access Team. We can accept referrals for those aged 18 and above, who have housing-related needs and are living in the county of Gwynedd.
