About the project

The Mental Health Supported Living Service project is for adults with a a mental health diagnosis who would like to live independently in their own home. We offer this support to anywhere in Pembrokeshire.
Supported living enables an individual to live independently while ensuring they are safe and supported according to their specific needs.
The support offered:

  • Enables individuals to maintain their own tenancies reducing the risk of homelessness.
  • Promotes engagement in meaningful activities within local community, social activities, and seek opportunities in volunteering, training, education.
  • Empowers individuals to ask for support & help to prevent relapses.
  • Allows for individuals to live independently with minimum support.

Eligibility / Referral Process

We offer supported living in partnership with Pembrokeshire County Council throughout Pembrokeshire.
You can access supported living support with funding directly from your local authority. Your Social Worker or Community Psychiatric Nurse (CPN) can refer you to Pembrokeshire County Council.
If Adferiad Recovery is identified as the preferred support provider, we will then contact the individual to arrange a visit, plan with the person how to meet their identified needs, agree start date of support.
